ticking 6speed transmission

Only makes this sound only under load and only to 4th gear. This is on 2003 3500 stock p/u, anyone else been thru this? and no there is nothing wrapped around the drive line.

You sure its the transmission? I know after a oil change the Typewriter tick can be heard from the engine
