I've seen a lot of this happen to the C3 piston but this is the first time a C4 has ripped. Allison has had a lot of problems with their pistons.

Finally a picture of the C4 apply piston. Couldnt get my "smart" phone to upload via tappatalk. 
I'm very excited to get the trans back together after fighting a issue I never even could have thought was the sole root of my problems. We don't know how or when this happened, but, this trans has had people touch the tuning that shouldn't have. (none of the tuners here) I didnt own the truck at the time, so it didnt really matter to me, but I see how serious it can be.
My initial fear after seeing the internal filter half full of metal was the P3 hub and the splines... but, I was wrong, and I'm glad I was. Could have turned out worse, but ill take a step at a time with the new stuff thats going in it.

I had this happen in my 01 roughly 2 years after it was built. Also had one in an lmm do the same about 20k after I built it as well, both of them the piston looked just like the picture of brians. However it wasnt completing shifts at all, well at least the 1-2 shift.

Yes the c 4 piston. Broviak tuning in one and duramaxtuner in the other
