    IMHO.... the ignition is the only thing it could be. If the airdog is run on a key on power and when the truck won't start the airdog is not running, the ignition switch is stuck in the off position.

    How much heat are you building on the line? I found sitting on the brakes until I had 1100* on my egt then rolling out would result in the turbo lighting every time.

    With the ficm plugged in and the glow plugs out you will see fuel coming out every time the injector fires. Sounds like something is disabling your injection.

    You will need a puller. They put a Teflon paste on the splines that over time turns into cement and is a pain to get off.

    Could be your valve lash causing the bad balance rate. Is it burning coolant or coming out the expansion tank?
